Documents You’ll Need (All Grades K–12)

Bring originals or official copies of the following:

• Proof of Age
  • Birth Certificate
• Proof of Residency (must be current)
  • Utility bill (electric, water, gas)
  • Lease agreement or mortgage statement
  • Louisiana ID with current address
• Parent/Guardian Photo ID
  • Driver’s license or other government-issued ID
• Immunization/Shot Records
  • Must be up to date per Louisiana Department of Health
• Social Security Card (optional but helpful)
• Custody Papers (if applicable)
  • Legal documents for guardianship or custody agreements
• Academic Records (if transferring)
  • Last report card, transcripts, or withdrawal form from previous school 
• IEP/504 Plan if applicable

Pre-K & Kindergarten

• Must be 4 years old (Pre-K) or 5 years old (K) by September 30
• Proof of income may be required for Pre-K programs (for eligibility)
• Schedule a kindergarten screening or readiness test if offered
• Ask about aftercare or early childhood services

Grades 1–5 (Elementary)

• Bring last report card or transfer form from previous school
• Ask about:
  • Bus transportation routes
  • School supply lists
  • After-school care programs
• Prepare your child for routines and meet-the-teacher day

Grades 6–8 (Middle School)

• Ensure transcripts or report cards are available for accurate placement
• Ask about:
  • Electives (band, art, PE, foreign language)
  • Locker policies and dress code
  • Clubs or extracurriculars
• Middle school may require a student orientation

Grades 9–12 (High School)

• Bring transcripts and any available test scores (ACT, LEAP, etc.)
• Discuss graduation requirements, career pathways, and dual enrollment options
• Ask about:
  • Course selection or scheduling meetings
  • Sports and extracurricular sign-up
  • Student ID requirements and dress code
