Documents You’ll Need (All Grades K–12)
Bring originals or official copies of the following:
- Proof of Age
- Proof of Residency (must be current)
- Utility bill (electric, water, gas)
- Lease agreement or mortgage statement
- Louisiana ID with current address
- Parent/Guardian Photo ID
- Driver’s license or other government-issued ID
- Immunization/Shot Records
- Must be up to date per Louisiana Department of Health
- Social Security Card (optional but helpful)
- Custody Papers (if applicable)
- Legal documents for guardianship or custody agreements
- Academic Records (if transferring)
- Last report card, transcripts, or withdrawal form from previous school
- IEP/504 Plan if applicable
Pre-K & Kindergarten
- Must be 4 years old (Pre-K) or 5 years old (K) by September 30
- Proof of income may be required for Pre-K programs (for eligibility)
- Schedule a kindergarten screening or readiness test if offered
- Ask about aftercare or early childhood services
Grades 1–5 (Elementary)
- Bring last report card or transfer form from previous school
- Ask about:
- Bus transportation routes
- School supply lists
- After-school care programs
- Prepare your child for routines and meet-the-teacher day
Grades 6–8 (Middle School)
- Ensure transcripts or report cards are available for accurate placement
- Ask about:
- Electives (band, art, PE, foreign language)
- Locker policies and dress code
- Clubs or extracurriculars
- Middle school may require a student orientation
Grades 9–12 (High School)
- Bring transcripts and any available test scores (ACT, LEAP, etc.)
- Discuss graduation requirements, career pathways, and dual enrollment options
- Ask about:
- Course selection or scheduling meetings
- Sports and extracurricular sign-up
- Student ID requirements and dress code
