Documents You’ll Need (All Grades K–12)
Bring originals or official copies of the following:
  • Proof of Age
    • Birth Certificate
  • Proof of Residency (must be current)
    • Utility bill (electric, water, gas)
    • Lease agreement or mortgage statement
    • Louisiana ID with current address
  • Parent/Guardian Photo ID
    • Driver’s license or other government-issued ID
  • Immunization/Shot Records
    • Must be up to date per Louisiana Department of Health
  • Social Security Card (optional but helpful)
